Practitioner Partner Program

Offer Functional Medicine, Botanical Medicine, and Evolutionary Nutrition Support for your clients in a collaborative care model.

Generating new leads and revenue as a health and wellness practitioner is not always easy. Evocentrix is looking for practitioners like you that we can send new clients, while also giving you the opportunity to be paid for time spent in clinical collaborative support with our team as we work together to provide the highest quality of care.

We all know or will meet someone that experiences some level of health challenge or concern in their lives. The Evocentrix programs address part of those challenges or concerns. You address the other part.

Here's how it works for you as a practitioner:

Receive referrals from us who need your services, either in-person, or via online care.
Introduce people with health challenges or concerns that can't be fully addressed through your service (you can schedule a call here to discuss what that looks like).
You continue to be your client's practitioner as part of their involvement in the program.
Receive a 10% collaborative care fee for the connection for your clients or referrals who enroll in our program as you collaborate in their care.
Support a vision to put sustainable health in the hands of everyone on the planet, and disrupt the healthcare system as we know it.
